• BI (Bibliographic Instruction) Evaluation  Use this form to evaluate a subject-specific DATABASE searching session given by a librarian
• Faculty Information Literacy Instruction & Assessment Plan: Faculty/Instructor fills out prior to scheduling a Subject-specific BI (Bibliographic Instruction) session with a librarian.
• Library Orientation Survey Use this form to evaluate a Student Orientation/Tour given by a librarian 
• Library Questionnaire - gives instructor an idea of how familiar students are with the library.
